Commercial Insurance and Medicare
New Jersey Dermatology and Mohs Surgery, PC participates with most major commercial insurance plans and Medicare. However, insurance networks can vary, so we strongly recommend confirming with your insurance carrier that we are in-network before your appointment.
As part of our agreement with your insurance company, our office will submit claims on your behalf. Any remaining balance after your insurance processes the claim becomes your responsibility. These patient-responsible costs may include co-payments, co-insurance, and deductibles. Co-payments are due at the time of check-in.
Please review your specific health plan or contact your insurance provider if you have questions about coverage or services that may require prior authorization.
Referrals
Certain insurance policies require a referral or authorization from your primary care provider before covering dermatology services. Please verify your plan requirements prior to your visit.
If your policy requires a referral, it must be received by our office before your appointment. Without a valid referral on file, we may need to reschedule your visit.
Our staff is happy to assist you if you have questions about insurance eligibility or referral requirements.
Self-Pay Patients
If you do not have insurance coverage, or if we are considered out-of-network with your plan, you are welcome to be seen on a self-pay basis.
Payment for the office visit portion will be collected at check-in. If additional procedures (such as biopsies, cryotherapy, or excisions) are performed, there may be additional charges — all of which will be discussed with you before proceeding.

Note: Cosmetic procedures are not covered by insurance and are considered self-pay, as they are deemed not medically necessary.
Accepted Forms of Payment
For your convenience, New Jersey Dermatology and Mohs Surgery, PC accepts the following payment methods:
- Debit cards
- Credit cards (Visa, Mastercard, Discover, and American Express)
- Cash
- Check
- HSA and FSA
Please be prepared to pay co-pays and known balances at the time of your visit.
Credit Card on File
We require patients (with the exception of those with Medicare and supplemental coverage) to keep a credit card on file to cover any remaining balance after your insurance company processes the claim. This card may also be used for cancellation or no-show fees in accordance with our office policy.

Pathology and Laboratory Fees
As part of your dermatologic care, Dr. Freedman may need to perform skin biopsies, bloodwork, or cultures. We try our best to always use dermatopathology services covered by your insurance and typically use Quest Diagnostics or LabCorp for laboratory testing.
These laboratories will bill your insurance directly for their services, as they operate independently from New Jersey Dermatology and Mohs Surgery, PC.
If your insurance requires use of a specific lab or diagnostic provider, please notify our office before your visit so that we can direct your specimens accordingly.
